Confirmation of your employer's financial support (or lack of support) is required.- Download, print, and complete the SDMS Foundation Certification Examination Grant - Employer Affirmation form (Click Here to Download).
- Ask your employer to review and sign/date the form.
- Scan the signed form (preferably in PDF, but PNG, JPG, or TIF are accepted).
- Upload form when prompted during the application.
Please be sure the required attachment is in one of the following formats (PDF preferred): PDF, JPG, or PNG. Maximum file size: 5 MB
